Location: New York, NY
Reports directly to the VP of Nursing Home Admissions and his/her designee, reviews Patient Review Instruments (PRI) to evaluate applicants for admission to the Home's inpatient services and facilitates the admissions process by serving as a liasion between Jewish Home staff and applicants, hospitals and community agencies.
Reviews PRI's and all supporting clinical documentation for appropriateness to the facility.
Serves as liaison to hospitals throughout the metropolitan area, making field visits as needed and telephone consultations to confer with hospital staff and to seek out and evaluate potential applicants for admission to Jewish Home Lifecare.
Completes all required clinical pre-admission assessment forms on all potential applicants evaluated, gathering additional medical and social information as needed for a thorough assessment.
Begins the application process during field visits and via telephone providing information about Jewish Home programs; may gather psychosocial and financial information to complete applications with applicants and/or their families.
Documents all applicant clinical and psycho-social assessments, visits hospitalized JHH residents, consultations and meetings submitting monthly statistics and reports as requested by the VP of Nursing Home Admissions and his/her designee.
Works with Managed care companies to obtain pre-authorization; reviews level requirements to obtain appropriate level prior to admission.
Arranges and conducts facility tours with social workers, case managers and other staff from our hospital partners.
Interacts with HMO Case Managers prior to admissions, to review skilled needs and maximization of level.
Skills/Requirements
BSN degree preferred, valid NYS RN license required.
PRI certification preferred
Experience in health care setting required. Previous experience in post acute care admissions process preferred.
Home Care Experience required.
Acute care experience a plus.
Bilingual (Spanish/English) a plus.
